Assignment Alert Form

We offer the email form below as a new way to improve communication between teachers and Library staff members.

Teachers are encouraged to alert Library staff to upcoming assignments and projects. This form provides a simple way of preparing the Youth Services Staff to effectively assist students when they visit the Library. Providing Library staff with advanced notice of assignments will allow us to reserve books on a subject and will enable us to be fully prepared when interacting with your students.

If you are using Internet Explorer 10 you will need to open this site using Google Chrome or FireFox in order for the form to submit. If you experience any other difficulty in submitting this form please contact us at (812) 537-2775 ext. 2.

Please fill out the online form below at least one two weeks prior to the assignment being given. You may also download and print a copy of the Assignment Alert form to fax, mail or drop off at the Lawrenceburg Public Library.
